Utilizing the Claims Database

HID’s Disease Management Program stands on a foundation of the Pharmaceutical Claims database. The database is used to ascertain which patients are at high risk and would benefit from management/intervention. In addition, the database is used to measure outcomes. The Management program can be augmented by a Prior Authorization program and/or Formulary development. These parts of the comprehensive program can fit together to enable Health Information Designs to identify the patients who would most benefit from disease state management and case management.
